All the Swiss cheese holes came together at the same time Why were the CRJ and heli on different frequencies I believe this was the main problem The CRJ knew nothing about the heli The TCAS was ineffective at that level Traffic separation in close vicinity of an airport is a joint pilot/controller responsibility Why was emergency traffic not given to the CRJ? Whole thing has too many faults

Here's a clear traffic control command that needs to be implemented into standard flight control lingo involving helicopters: it shall be the chopper's call sign followed by the letters , ' E-HDS' which stands for ( Emergency - Hover- Descend, Safely) eg (PAT-25, EHDS) This means to immediately halt, hover, and descend to your lowest safe altitude until cleared to proceed This order is only to be used in VR flight conditions It isshall be the chopper's equivalent to a fixed wing go ɓaround
